First Appointments

When making an appointment for the first time please contact our office staff at 630-305-0464, ext. 300, and have your insurance card available.

Use the Client Portal at portal.therapyappointment.com to complete your online registration forms. If you have not received an email invitation to set up your Client Portal, please call our office to initiate this. We would appreciate it if you would complete your online forms 48 hours prior to your appointment.

What To Expect When You Call Our Office

When you call our office you will be asked a few questions to get you started with setting up your initial visit. At this time you will have the opportunity to inquire about the providers here at Spalding Clinical Services, and our policies and procedures.

You are responsible to know the mental health benefits on your insurance plan; therefore, we ask that you call your insurance prior to your first visit to verify the following information:

  • That the provider you would like to see is in your insurance network.
  • What the mental health benefits are for your insurance plan.
  •  If an authorization or a referral is needed and, if so, to obtain one.

You may complete your registration paperwork online and bring it with you to your first appointment. However, if you do not wish to complete it online, please arrive 20 minutes early to complete it in the office.

Listed below are some of the insurance companies with which the providers may be contracted. Each provider at Spalding Clinical Services is contracted individually with insurance plans. Please keep in mind that you should always contact your insurance to verify whether the provider you have chosen is in your network.

Payment Information

We will gladly bill your insurance for services provided to you if we have the necessary information. Please bring your most current insurance card with you for each visit.  You are responsible for giving us full and accurate insurance information (including secondary, if applicable) at your first visit and immediately thereafter whenever there are any changes with your insurance coverage.  If this information is not given in a timely manner and claims are denied, you will be responsible for the balance due. Co-pays are to be paid at the beginning of each visit. If you are paying by check please make it payable to the clinician that you see.

Clinicians that accept payment by credit or debit cards are: Kris Munson, LCPC,  Carrigan Manetti, Psy.D,  Keith LeVasseur, LCPC,  Mary Beth McLaughlin, M.A. LCSW,  Lauren Kunstler, LCSW, and Lisa Gray, LCSW.

If you are seeing one of these clinicians, please complete the Credit Card Authorization Form and bring with you on your first visit. All other clinicians accept payment by cash or check only. All of the clinicians at Spalding Clinical Services are independently licensed with the State of Illinois.  Each clinician is contracted individually with insurance companies, not as a group, so they may not be in the same provider network. Missed Appointments/Cancellations Please make sure to arrive on the correct date and time of your appointment.  The clinicians set aside 45-50 minutes for your appointment; therefore, we require that you give us 24-hour notice if you are not able to keep your appointment so they have adequate time to schedule another client.  Missed appointments and cancellations with less than 24 hours notice will be charged to you in full. This charge cannot be billed to your insurance company.

Good Faith Estimate Notice

Notice to clients and prospective self-pay clients:

Under the law, health care providers need to give clients who do not have insurance or who are not using insurance, an estimate of the expected charges for their healthcare services.

You have the right to receive a Good Faith Estimate for the total expected cost of your psychotherapy services if you will be, or are currently, a self-pay client.

You can ask your clinician for a Good Faith Estimate before you schedule a service, or at any time during treatment.
